چکیده

Among I70 patients with Adams-Stokes syndrome, the majority had sinus rhythm between attacks. Clinical and electrocardiographic features of 67 patients in whom syncope was caused by paroxysmal third-degree atrioventricular block are discussed. The patients had normal pulse rates between attacks, their cerebral symptoms were usually not accompanied by cardiac symptoms, and 25 per cent had no symptoms suggesting heart disease. However, in 83 per cent the electrocardiogram showed constantly interventricular conduction disturbances and 7I per cent episodically atrioventricular conduction disturbances. It appears that the diagnosis, which otherwise may be missed, may be greatly facilitated by electrocardiograms. The variability and complexity of the QRS patterns suggest a development of third-degree AV block in the form of an 'additive' block. The final lesion in the complete block may vary both in location, proximall distal, in the conduction system, and in degree of involvement of the pacemaking tissue. Both factors may contribute to an alternation between episodes of third-degree AV block with and without escape rhythm, and correspondingly without and with Adams-Stokes attacks.

Bilateral Bundle-Branch Block

The electrocardiograms and salient clinical features of seven patients with electrocardiographic evidence of various types of bilateral bundle-branch block (BBBB) have been presented. When the complexes change in form and the P-R interval varies, the mechanisms are usually best interpreted by using the concepts of first degree, second degree, and third degree bundle-branch block with or without...

Syncope and paroxysmal atrioventricular block

Current literature reveals three types of paroxysmal atrioventricular block (AVB) that can cause syncope: Intrinsic paroxysmal atrioventricular block is due to an intrinsic disease of the AV conduction system; this type of "cardiac syncope" is also called Stokes-Adams attack; Extrinsic vagal paroxysmal atrioventricular block is linked to the effect of the parasympathetic nervous system on cardi...

Congenital Aneurysmal Defect of the Mem - branous Portion of the Ventricular Septum Associated with Heart Block , Ventricular Flutter , Adams - Stokes Syndrome and Death By RICHARD

A case of congenital anieurysmal defect of the ventricular septumi is reported which appears to be unique in that this lesion was the only significant finding at autopsy. The patient had evidence of auriculoventricular block for a period of 26 years, at first partial and then complete. She succumbed at the age of 47 to a series of Adams-Stokes attacks which were demonstrated to be set off by pa...

Paroxysmal Heart Block in Bundle Branch Block.

When a patient is suspected of having Stokes-Adams disease the electrocardiogram is of critical importance, particularly in respect of the auriculo-ventricular (A-V) conduction time. Should this not be prolonged, the possibility of paroxysmal heart block remains, although the diagnosis of Stokes-Adams disease is uncertain without some degree of heart block recorded graphically. It is not known ...

Clinical commentary Unrecognized paroxysmal ventricular standstill masquerading as epilepsy: a Stokes-Adams attack

Recognition of cardiac syncope masquerading as epilepsy may be difficult in the Emergency Department. We report a middle-aged man with recent onset convulsions who posed a diagnostic puzzle before it was found that he had paroxysmal ventricular standstill with complete atrioventricular block: he made a complete recovery after temporary pacemaker insertion. The main lessons from this case were (...
